Family Information	Father's Name Mother's Name Street Address City ZipCode Home Phone Email Address Cost: $100 per child per year for child of current member of the Bharatiya Temple of Lansing, $200 for child of Non-member.  Requests for fee waiver will be considered on an individual basis. Requests should be forwarded to the Board of Trustees of the temple.  All fees go directly to the Bharatiya Temple of Lansing. Instructors do not receive any compensation from the temple. The Bharatiya Temple of Lansing is a charitable organization established under IRC Sec. 501(c)(3). Therefore, donations and other fees, to the extent allowable under the Code, are deductible for tax purposes. Please contact your tax advisor for additional clarification. Registrations received after September but before March 1, are at full cost and good until the end of the Bal Vihar year (August). Registrations received after March 1 are at half cost and good until September of the same year at which time re-registration at the full cost is necessary. To accomodate the differences in the membership status and rates, the rates will be determined as of the membership status at registration.
Waiver and Release	In consideration of your accepting my child's participation in the above program, I hereby, for myself and my child, waive and release The Bharatiya Temple of Lansing, and their officers, trustees, volunteers and members, and all other persons participating in the program, or involved in planning or execution of the program, from all liability or claims arising from any injury to myself, my child or my property.
